Provider will travel to patient's home for acute or chronic visit, if home visit is to establish care extra $50 charge will apply

Monthly membership includes 2 clinic or telehealth visits, each additional visit $40 acute or chronic visit, no charge for nurse visits, 10% off all med spa services and priority scheduling
Nurse Visit
$40
These are visits where the patient will only see the nurse and not the provider, this includes coming in for a vital sign check, vaccine or injection, basic wound check or dressing change, lab draw or specimen collection
Sick Visit
$65
Acute care visit: sore throat, upper respiratory infection, sinus infection, Urinary tract infection, etc
